We work with primary care organisations to help extend access to services and offer more complete and integrated care.
We do this by working with groups of GP practices to create DLS Access, a Health Hub that connects patients with their doctors and enables care to be delivered digitally. Patients contact the Hub using a telephone, website or app. Doctors call them back within an hour by phone or by Skype.
In 70 per cent of cases, this is all the patient needs. The rest are offered a same-day face-to-face appointment. This efficiency makes it possible to offer services 8am-8pm and at weekends where required.
The Health Hub environment also means social, community, mental health and acute services can be integrated into primary care provision. This creates a more integrated experience for patients as health and social care professionals can be accessed through the same system and see the same records. NHS 111 patients can be referred into the Hub too.
DLS Prescribe, DLS Care Planning and DLS Connect offer patients help in managing their longer term care needs digitally, through the hub. Whilst our newly developed virtual clinics mean healthcare professionals can monitor patients with long term conditions remotely and only seen when their data shows cause for concern.
